Applying for a companion card

WHO IS ELIGIBLE TO APPLY?

Residents of South Australia who have a significant and permanent disability and who need attendant care support, can apply for a Companion Card.

When applying for a Companion Card you will be assessed against our eligibility criteria. To be eligible for a Companion Card, you must demonstrate all of the following:  

  • you are a permanent Australian resident, living in South Australia;
  • you have a significant and permanent disability;
  • that due to the impact of the disability, you would be unable to participate at most community venues and activities without attendant care support; and
  • the need for this level of attendant care will be life-long.  

Attendant care support means you require significant assistance with mobility, communication, self-care or planning and problem solving where the use of aids, equipment or alternative strategies do not enable you to carry out tasks independently. Attendant care support does not include providing reassurance or encouragement nor can it be for infrequent or unexpected events or medical emergencies.

HOW DO I APPLY?

You must fill out an application form, and post it to the SA Companion Card team at Companion Card, GPO Box 292, Adelaide SA 5001.

The application form contains further instructions on how to apply for the Companion Card. Please note you will need to provide two identical, passport quality and sized colour photographs with your application.

  • downloading and printing a form

  • writing a letter requesting a form to: Companion Card, GPO Box 292, Adelaide SA 5001.

WHAT SUPPORTING INFORMATION SHOULD I SEND WITH THE APPLICATION?

You should send any relevant supporting information, such as:
  • contact details of support services (e.g. day programs, respite services);

  • contact details for up-to-date reports from health professionals (e.g. a specialist, psychologist, speech therapist, physiotherapist, paediatrician, occupational therapist, registered nurse or social worker); or

  • any other information that you believe will show that you qualify for a Companion Card.

WHAT CAN I DO IF MY APPLICATION IS DECLINED?

In the first instance you should contact SA Companion Card on 1800 667 110 and speak to  a Companion Card Customer Services Officer to discuss the decision.

You may be advised to submit new information that more clearly demonstrates you meet the Companion Card eligibility criteria.

If your application is still declined, you can ask for a review by the Companion Card's External Appeal Committee by writing to:

Any new information you provide, along with the information obtained in your original application, will be sent to the External Appeal Committee to consider. They will write to you with their decision which is final.
